Differential diagnosis
Here is a list of differential diagnosis that needs to be ruled out before setting a PANS/PANDAS diagnose.
And now I actually wonder why Rheumatic fever isn't on the list. Sure it has been rare with the antibiotic era but since 10 years, with the antibiotic resistance fear, they rarely put children on antibiotics for a strep throat. They don't even take strep tests.
What tests has been done to rule the list below out?

Other possible complications that may correspond with some of Arvids symptoms (specially the rash):
And / or: (the hair analysis showed high levels of natrium (sodium) and kalium (potassium) and low ratio which could point to problems with the kidneys or chronic infections)
- Acute proliferative glomerulonephritis
- Poststreptococcal glomerulonephritis

The school medicine of today can't do this analysis in ordinary Swedish healthcare, there need to be a specialist team gathered I think. If it's not obvious what you suffering from and they don't think you will die tomorrow the health care in Sweden, at least, is like fumbling in the dark. It may be the autoimmune diseases that is the hardest for them to handle.
Bartonella?

Found this mark on Arvid in autumn 2016. Rather weak marks with abit hardend skin. It didn't itch or hurt. Didn't thought it had with his sickness to do. After the IVIG in may 2018, only hours after we got home, it was very red and hurting, at this time he hadn't complained about this at no times since I noticed it. I took on some coconut oil with drops of frankincense oil in. The day after it didn't hurt him no more and was not that red anymore.
Could this togethter with his other symptoms point to Bartonella?

Below are some updated pictures from May 2019 of the mark on his foot. This together with all his other symptoms could instead be bartonella, or with an PANDAS/Reumatic fever from the beginning 2012.
He got Doxycyclin for 10 days in July 2018, got some herxing for 5 days and then better than before for 5 days, but worse again when the 10 days was ended. 10 days are not enough for Bartonella.

With Gentamicin + doxycycline 14 + 28 days 7/7 patients recovered.
